REQUIREMENTS FOR APPLICATION OF MARRIAGE LICENSE
1. Pre-Marriage Counseling seminar for 2 days
First day - DSWD
Second day - Local Civil Registry and MHO Schedule of seminar is every other Tuesday
and Wednesday. Applicants must be both 18 years old of age.
2. Birth Certificate of applicants 1 original copy and 2 pcs authenticated photo copies and
readable, no alternations if tampered secure NSO copy.
3. Applicants must be a residence of PDAS ( it may either both or any male or female)
Marriage Contract of both parents 1 copy original and 2 pcs. authenticated photo copies
and no readable, no alterations 18 to 25 yrs old must seek consent/advice of parents
4. 26 yrs old and above CENOMAR- Certificate of No Marriage at NSO Butuan City
5. If applicants are widow and widowers must present Death Certificate of the deceased
Husband or Wife Applicants who are member of the Armed Forces of the Philippines and
PN are required to submit Permission to marry from the Commanding Officer / Personnel

Payment is P 350.00 plus miscellaneous
